Job Summary:

Implement programs to modify cardiovascular risk factors through exercise training, multidisciplinary education and counseling for individuals with known cardiac disease and for their families. Assist cardiac individuals in attaining an optimal level of physical, psychosocial and vocational functioning.

Key Position Details:

• Candidates who have completed a cardiac rehab internship will be considered.
• Position will include shifts at both Mercy and Unity campuses. This includes inpatient cardiac rehab (Mercy only), outpatient cardiac rehab and PAD program across campuses.
• These will be 8-hour shifts with start times varying between 7:30 am - 9:00 am and will end between 4:00 pm and 5:30 pm.
• Weekend and holiday shifts are not required for the casual role however can be picked up when they are available.

Job Description:

• Assess physical condition of patient and evaluate patient for graded exercise.
  • Contact patients in a timely manner after receiving referrals.
  • Educate patient on what services are available to them.
  • Document heart/lung sounds, musculoskeletal problems or concerns and status of incision site.
  • Review medical history to assess problems with beginning physical activity.
• Design and implement individualized exercise or activity plans for each assigned patient.
  • Identify learning needs of each patient.
  • Report progress on the patient care plan.
  • Instruct the patient and family on appropriate home exercises/activities.​
• Monitor progress during exercise sessions.
  • Analyze and interpret clinical signs and symptoms along with exercise EKG strips during the sessions.
  • Adjusting the exercise plan accordingly
  • Follows correct procedures in an emergency situation.
• Patient Education
  • Assess patient’s family’s knowledge concerning cardiovascular risk factors.
  • Develops and implements, in collaboration with clients and individual, realistic education plan.
  • Conducts classes on heart disease and risk factor modification.
  • Develops and utilizes cardiac rehab educational materials.
• Consult/collaborate with physicians and other health professionals regarding patient care, cardiac rehab education and program development.
  • Communicates with patient’s physician and/or other health care professional as needs dictate.
  • Participates in meetings with medical director for patient care and quality improvement initiatives.
  • Maintains department statistics and reports.*
  • Develop ongoing programs to deliver cardiac rehabilitation services.
  • Other duties as assigned.

Physical Demands

Medium Work*: Lifting weight Up to 32 lbs. occasionally (*Allina Safe Patient Moving Policy), Up to 25 lbs. frequently.

Job Requirements- Preferred

Master's degree Exercise Physiology, Nursing or Occupation Therapy preferred ACSM Exercise Specialist - Kronos Skill preferred.

Experience in cardiac rehab required Certified Occupational Therapy Assistant - National Board for Certification in Occupational Therapy required if occupational therapist upon hire - or - Licensed Registered Nurse - MN Board of Nursing required if registered nurse upon hire. CPR - Multisource required upon hire. ACLS/BLS Tier 2-Advanced Cardiac Life Support - Multisource required upon hire.
